When a prospective client completes the ‘Contact Faye’ enquiry form on the website, the following information is collected and sent to Faye via email:
- Full name
- Contact telephone number

- Any further information that the prospective client chooses to share via the ‘additional information’ option
This information is stored on Faye’s private email address. No other persons have access to this email account.
- Consultation Card
At the start of every initial treatment, the client will be required to complete a consultation card. The client will be expected to detail the following information:
- Full name
- Age
- Date of birth
- Contact telephone number
- Email address
- Contraindications to the treatment
- Current medication
- Recent injury history
Consultation cards are stored in a locked cabinet which is only accessible to Faye.
Information is never collected from or shared with third parties.

Consultation cards are kept for up to 4 years post treatment. They are then destroyed by use of a shredder.
